A health-tech platform that put patient records where clinicians could actually reach them

The challenge

Patient information lived in paper folders, personal spreadsheets and the memories of individual staff. When a clinician needed history, someone walked to another room. Records went missing during shift changes, and management had no reliable view of volumes, outcomes or resource use.

What we did

We worked through this in stages, starting with what had to be true before anything else could be built.

1. Ran discovery on the ward rather than in a boardroom, shadowing clinicians through a full shift to map where information was actually needed.
2. Built a role-based records platform with an audit trail, designed for low-bandwidth conditions and tablets in use on the floor.
3. Migrated existing paper records in staged batches so the clinic never ran without a working system during transition.
4. Trained every clinical and administrative user in small cohorts and left a super-user in each department.
